This is a great example in this 53-year-old of obtaining a more balanced breast. This patient had 375cc saline implants placed in 1999. She states they never really dropped down and she’s always been higher on the right side and her implant more displaced towards her under arm. She was now interested in obtaining a better shape, going to silicone implants and going larger but at the same time doing all we could to avoid a lift, accepting that her nipples may still be a hint low. At surgery I sutured the under arm area of her capsule to encourage the new implant to move down, performed aggressive capsulotomies in the lower pole and placed Allergan style SCM gummy bear implants, 560cc on the right and 600cc on the left all through peri-areolar incisions, with a much better balanced result.
