Breast Mastopexy

This 41-year-old patient is a great illustration of a few principles. She had original augmentation surgery in 2004 with saline implants over the muscle. She felt too low and her saline implants were too slushy. She wanted to be lifted, more fullness in the upper pole and change to silicone. We decided to not only change her implants (Allergan 310cc) and do a lift but move them to under the muscle as well.

This 35-year-old female had lost breast volume following breast-feeding. She now desired a C cup and very natural wanting to avoid that high riding breast look. Her rib cage showed an indentation at the lower sternum and flaring of her ribs on the lower left side. She was also slightly larger on the right than the left. I used a 300cc Allergan style SRLP silicone implant kept completely submuscular with a Brazilian lift on top of that. her results are shown one year later still holding a very full, natural look.

This 44-year-old woman felt her breasts just did not fit her body tight. She was a DD and felt very droopy. She wanted a smaller breast size and lift as well but not interested in an implant. A Brazilian type breast lift was performed and her results, eight months later show a much more athletic look, which was her goal!

This 42-year-old is a classic example of a breast implant revision going from an above muscle breast augmentation years ago to an under the muscle implant exchange with a lift. Her original implants were 350 mL textured silicone implants above the muscle and over the years her skin and stretched quite a bit. She was now complaining of a hanging sensation as well as loss of volume in the upper pole. She really didn’t want to be much smaller, just more support and better cleavage. In surgery, I removed her old implants and opened the space under her muscle.

Textured implants have received much attention lately and deservedly so. Research continues but I thought I would post a reason as to why we use them. One thing they offer in many patients is an increased ability to stay where you put them. This 67-year-old female was unhappy with her drooping breasts and desired to be a good D to DD. Keeping implants and breast tissue uphill is always a challenge in mastopexy augmentation and even more so in this patient with loose skin.

This patient had undergone a lift with implants three years prior. Unfortunately, she felt too large and that her implants, being high profile and 425 cc, were too big and she still felt too droopy. We went to surgery and changed her implants to 330 cc Allergan silicone gel and did a Brazilian type lift, which specifically removes much of the droopy tissue from the bottom of her breasts. She also has a naturally occurring indentation of her lower breast bone which is somewhat common. Her results now two years later, show her breasts still up off her tummy and passing the pencil test!

This 61-year-old patient is a great example of a revision surgery that is becoming much more popular. She had saline implants placed 12 years prior, 450cc on the left and 400cc on the right. She felt now too low in both the position of her implants and breast tissue, wanting now to be smaller and perkier! We went to surgery and did work on all areas, replacing her implants with 360cc silicone, tightening her capsules, and doing a full breast lift, and 6 months later she is right where she wants to be.

This woman had her first breast augmentation at age 25 with 250cc saline implants over the muscle. Now 38 years old, three kids later and an active tennis player, she felt that her breasts had dropped and that she had lost upper pole fullness. At surgery, we removed her old implants, put 310cc Allergen silicone gel implants under the muscle with complete muscular coverage to prevent this implant from slipping too low or back to the previous space. This was followed by a Brazilian, superior pedicure full mastopexy and 16 months later her shape is great!
