This young woman is a great example of correction done through a transaxillary or underarm approach. Her original augmentation was done six months prior by a non-plastic surgeon who used the underarm approach but did not use the endoscope. Using a blind approach, they were not able to adequately open the lower pole of the breast or release the lower portion of muscle. This caused a very high riding and unnatural look. Going back in through her underarm and using the endoscope, I removed her 350cc high profile implants and performed bilateral capsulotomies. I then placed 475cc silicone gel high profile implants using a Keller funnel. She now shows much more volume below the nipple, as a breast should and on the side view, a much more natural slope.

This woman had her first breast augmentation at age 25 with 250cc saline implants over the muscle. Now 38 years old, three kids later and an active tennis player, she felt that her breasts had dropped and that she had lost upper pole fullness. At surgery, we removed her old implants, put 310cc Allergen silicone gel implants under the muscle with complete muscular coverage to prevent this implant from slipping too low or back to the previous space. This was followed by a Brazilian, superior pedicure full mastopexy and 16 months later her shape is great!

This 48 year old woman was sent to me by another plastic surgeon. In 2002 she had saline implants placed over the muscle through a belly button approach. They had then developed a painfully hard Grade IV contracture. She was told by other surgeons it would be a multi-stage procedure but in one operation we removed her capsules and implants, put 325cc implants under the muscle and then performed a Brazilian lift on top of that. At 7 months post op she is pain free and thrilled!

This is a 51 year old who had 350cc silicone implants placed above the muscle 28 years ago! She now felt too large and was looking for a more natural look. At surgery, we placed Allergen Style 410 teardrop gummy implants 245cc, with a very natural result. And being above the muscle, these 410 implants were a great choice.

This 52 year old female presented with saline 325 cc implants placed 10 years prior. Now, she felt like she had too much droop and was hoping to correct that. I felt her implants were too low and too wide. I tightened the implant pockets and changed her to 339cc Style 15 Allergan gel implants and did a Brazilian breast lift. She is seen here at 3 months post op.

This 61-year-old patient is a great example of a revision surgery that is becoming much more popular. She had saline implants placed 12 years prior, 450cc on the left and 400cc on the right. She felt too low in both the position of her implants and breast tissue, wanting now to be smaller and perkier! We went to surgery and did work on all areas, replacing her implants with 360cc silicone, tightening her capsules, and doing a full breast lift, and 6 months later she is right where she wants to be.

This patient is a great example of adjustments that can be done through the under-arm incision. Many people think that once implants are placed this way that any further surgery must be an incision on the breast. This patient had her implants placed originally through the under arm and the right implant failed to settle at the same level as the other side. She was taken back to surgery for a capsulotomy all done through the under arm using the endoscope with great results!
