This young woman is a great example of correction done through a transaxillary or underarm approach. Her original augmentation was done six months prior by a non-plastic surgeon who used the underarm approach but did not use the endoscope. Using a blind approach, they were not able to adequately open the lower pole of the breast or release the lower portion of muscle. This caused a very high riding and unnatural look. Going back in through her underarm and using the endoscope, I removed her 350cc high profile implants and performed bilateral capsulotomies. I then placed 475cc silicone gel high profile implants using a Keller funnel. She now shows much more volume below the nipple, as a breast should and on the side view, a much more natural slope.
